A $5.00 fee will be incurred if billing for your co-payment is required. Our office and optical shop are wheelchair accessible and free indoor parking is available for all to use.

Please give 24 hours notice if you are unable to keep your scheduled appointment.

A $25.00 fee will be charged for all no show appointments.
